To request your scholarship for the March 15, 2012 Grant-Writing Workshop at St. Cloud State University, simply submit the request form below. We will respond to your request and advise on scholarship availability.

Scholarship Qualifications:
1. You must be a direct representative (Staff or Board Member) of a farmers' market or a farmers' market vendor.
2. The farmers' market or vendor you represent must be a current 2012 member with the MFMA.
3. Attendees must register & pay for the grant-writing workshop with St. Cloud State University directly. See attached flyer for registration.
4. The MFMA reserves the right to limit attendance to one person per membership. An additional interested attendees from your organization may be listed on this initial registration form for scholarship consideration should there be remaining scholarships available as of March 1st.
5. Submit proof of workshop registration by March 1st to hold your scholarship.
6. Upon completion of the grant-writing workshop, scholarship grantees will submit proof of attendance for a reimbursement of up to $79.00.
